Go to WordPress Dashboard > JetEngine > Forms <\/em><\/strong>and open\/add a form. <\/p>\n\n\n

Make sure that Forms (Legacy)<\/strong> are activated in the Modules tab of the JetEngine settings.<\/p><\/div><\/div>\n\n\n\n

Once the editor is opened, press the \u201cAdd Field<\/strong>\u201d button and the editing button with a pencil-shaped icon on it.<\/p>\n\n\n\n

Select one of the options in the Type <\/strong>field, give a Name <\/strong>to the filter, and complete the further fields. <\/p>\n\n\n

The Glossary<\/strong> data source can be used only for the Radio, Select, and Checkbox field types.<\/p><\/div><\/div>\n\n\n\n

In the Fill Options From <\/strong>field, mind choosing a \u201cGlossary\u201d option. Then, specify the item in the Select Glossary <\/strong>field.<\/p>\n\n\n\n

Complete the following adjustments of the field, and don\u2019t forget to push the \u201cApply Changes<\/strong>\u201d button.<\/p>\n\n\n\n

Make further customization to the form if needed and push the \u201cPublish\/Update<\/strong>\u201d button.<\/p>\n\n\n\n\"radio\n\n\n\n

Head to the page where you want to place the form. To do so, go to WordPress Dashboard > Pages <\/em><\/strong>and open one of the pages. The editing can be done both in Elementor and Gutenberg editors.<\/p>\n\n\n\n

Find the Form <\/strong>widget or block and place it on the page.<\/p>\n\n\n\n

In Elementor, it will look like that:<\/p>\n\n\n\n\"form\n\n\n\n
